What Is an Advance Directive?
An advance directive is the legal document that formalizes your wishes from advance care planning. It is your written instructions about your future medical care. The two most common types are a Living Will and a Durable Power of Attorney for Health Care (or "Healthcare Proxy").
What It Does
- Living Will: Specifies the types of medical treatments you would or would not want in end-of-life situations.
- Healthcare Proxy: Designates a person you trust to make medical decisions for you if you are unable to.
